I guess my answer can be broken down into two parts.We aren't really sure what the pull ratio is. So far, it seems to be: Holo Rare (1:4; 9 per box), EX and BREAK(1:8; about 4-5 per box per category), Full Art (about 1:16; about 2 per box), and Secret Rare (1:72; 1 per 2 boxes). Additionally, although BREAKs share approximately the same rarity and pull ratio, they occupy the same 'slot' as the reverse holo, so you can pull a BREAK and an EX in the same pack, skewing the ratio slightly.However, it should be known that boxes don't guarantee anything other than rares. Even for sealed boxes, boosters are still packed randomly, so there is no absolute guarantee that you will get anything, regardless of the ratio.
Logic dictates that you should get about 5 EXs per box. But is there a chance, however small it may be, that you could get a box with no EXs in it? Is there a chance that you could get 3 secret rares in just one box? Yes.Short answer is that while we know the approximate pull ratio, booster boxes are still random, and there is no guarantee that any box will follow this ratio.
It is highly unlikely that it won't in some way, but the chance is still there. The ratios of cards in a box depends on how they're distributed through a case (6 boxes). A case is specifically set with a set amount of each EX, hold etc (with an odd few extras since different sets have different amounts). Having opened many cases it's always been that the cards are pretty randomly distributed between the boxes in the case, there's no set amount that will be in each box so some boxes may have a really low a amount of EX/FA/BREAK for example but if so another box from the same case will likely have more than 'usual'. It's rare for any boxes these days to be really badly distributed and usually stay close to the observed standard amount but it's not specifically set.
I mean i believe the standard is more of an average, like you're basically gaurenteed a certain amount of ultra rares but you can get more, from personal experience the average seems to be at the very least 3 ex's and 1 full art per boxFor example, 2 weeks ago i opened an Ancient Origins Booster box and pulled a Kyurem EX, a Giratina EX, a Hoopa EX, a Mega Ampharos EX, a Mega Tyranitar EX and a Kyurem EX Full Art. So 5 EX's and 1 Full Art, Overall a great boxWhereas today i opened a Breakthrough Booster Box and pulled 4 Break cards, a Houndoom EX, a Mega Glalie EX, a Mega Mewtwo X EX and a Full art Brigette.

Overall an average box.I also watch alot of booster box openings and i dont think i've ever seen anyone pull less than 3 Ultra rares out of a Booster Box.Disregarding specifically my experience, Logic dictates that you should never get a box with no ultra rares. Think of it this way, Fates Collide has 48 possible Rares, 27 of those are Reg Rares/Holos, 11 of those are EX's and 10 are Full arts (21 Ultra rares).

In a Booster box, there are 36 Packs, assuming you pull every single holo (good luck with that) and reg rare in your first 27 packs, theres still 9 packs left. The chances of all of them being repeats are incredibly low.TL:DR I dont think Boxes are Fixed, My experience with booster boxes says that on average you should pull a certain amount of EX's (3+) and generally 1 Full art, but ultimately its down to luck. I recommend grabbing a booster box anyway because opening packs is alot of fun.
